AFFILIATED BUSINESS ARRANGEMENT DISCLOSURE NOTICE

The Crestway Group has affiliated business relationship with several business entities. Affiliate relationship means the relationship among business entities where one entity has effective control over the other by virtue of a partnership or other agreement or is under common control with the other by a third entity or where an entity is a corporation related to another corporation as parent to subsidiary by an identity of stock ownership.

The Crestway Group has 100% common control of the following business entities:

• Crestway Insurance Group, LLC
• Crestway Advisory Group
• Crestway Financial Group
• Crestway Mortgage Group
• Crestway Real Estate Group

Crestway Group affiliated businesses may share information between affiliated business entities for referral and/or business-related purposes. However, if referral to one or more of the Crestway Group affiliated business entities clients have the right to shop for these services and are not obligated to work with any of the Crestway Group affiliated business entities referred to you.
